1. 兩年年費共HK$100之劃線支票 (抬頭「通利琴行有限公司」或“Tom Lee Music Co., Ltd.” ) (現豁免首兩年會費)
2. 相關音樂考試証書副本
3. 三年或以上之教學證明 (如私人任教，請提供印有申請者姓名之學生相關考試證書)

Option 1:
Please fill in the e-application form of Hong Kong Orchestral Teachers' Circle via” Online Membership Application Form” page.

Option 2:
Download & fill in the PDF version , completed and send the application form to Hong Kong Orchestral Teachers' Circle at “11/F, Silvercord Tower 1, 30 Canton Road, Tsimshatsui, Kowloon” or return it to any Tom Lee Music branch with the followings:

1. HK$100 membership fee for 2 years should be paid with crossed cheque made payable to “Tom Lee Music Co., Ltd.” (Membership fee waived for first 2-year )
2. Photocopy of related music examination certificates
3. 3 or more years teaching proof (If private teaching, please provide the related certificates of students)

Option 3:
Please send the completed application form to Hong Kong Orchestral Teachers' Circle at “11/F, Silvercord Tower 1, 30 Canton Road, Tsimshatsui, Kowloon” or return it to any Tom Lee Music branch with the followings:

所提交之文件僅作審核資格用途，一經審核，申請者約於四星期內接獲通知。The supporting documents are for verification only, and the applicant will be notified within 4 weeks if the application is approved.

續會兩年年費HK$100 或 會員於會籍有效的兩年內(由發出日期起計有效兩年)於香港通利琴行購物累積滿港幣5,000元*，可在符合協會規定情況下，免除續會年費。HK$100 renewal fee for 2 years membership OR a member who has made HK$5000* accumulated purchase at Tom Lee Music Retail Branches in Hong Kong during his membership years (It is valid for 2 years from date of issue) is entitled to a waiver of renewal fee, subject to the rules of Hong Kong Orchestral Teachers' circle.
